Job Description

Somerfield at The Heritage is a 5-star, 66-bed skilled nursing facility on the beautiful campus of The Heritage at Brentwood in Brentwood, TN.

The Activities Partner assists with the overall planning and implementation of a varied activity program designed to meet the physical, emotional and spiritual needs of Health Center residents.

Essential Job Duties:

  • Reviews resident activity needs and plans total recreation program based on individual needs and resident interest.
  • Coordinates and provides recreation activities: reality orientation, current events, discussion groups, re-motivation therapy, sensory stimulation, cultural activities, religious services, special entertainment, out trips, special events, and one-on-one visits, as indicated.
  • Involved with the implementation of health center activities as assigned by the Activities Director.
  • Offers suggestions/ideas for upcoming program of activities to the Activities Director.
  • Performs and directs the movement of residents to and from recreational areas.
  • Prepares and maintains daily participation logs and schedules in accordance with state and federal regulations.
  • Maintains accurate documentation of residents’ progress and responses to program.
  • Assists with the purchases and inventories supplies and equipment. Inspects recreation equipment to determine repair and maintenance needs and cleanliness.
  • Assists with volunteer activities, enlists their support and coordinates their efforts; provides training and supervision of volunteers.

Other Duties:

  • Coordinates activity services with other departments.
  • Attends in-service training and education sessions, as assigned.
  • Performs specific work duties and responsibilities assigned by supervisor.

Somerfield Heath Center offers a variety of services including personalized short- and long-term care, rehabilitation services, skilled nursing, and memory care, if needed. Through team interventions we creatively work together to assist every resident with maintaining their level of independence by focusing on their strengths and capabilities.

QUALIFICATIONS: High school diploma or GED equivalent. Proficient computer skills using Microsoft Word or similar word processing software. Experience working with the geriatric population. Some experience in a social or recreational program in a licensed health care setting preferred.

PHYSICAL REQUIREMENTS/WORKING CONDITIONS: Frequent standing, stooping, bending, stretching, squatting for approximately 60% of work day. Must be able to lift and carry up to 50 lbs. occasionally. Must be able to push residents weighing up to 250 lbs. in geri-chairs and/or wheelchairs. May be exposed to communicable diseases, HIV and Hepatitis B viruses, industrial and household chemicals, offensive odors and combative behavior.
